Michael's comprehensive pre-program questionnaire enables him to better tailor his presentation to meet your needs. Please answer all relevant questions skipping over any which may not apply due to the nature of your event.

IMPORTANT: This questionnaire is Michael's first step in preparing for his presentation at your event. To ensure a program that is tailored to best meet your needs it is imperative that it be completed as thoroughly as possible and submitted at least 30 days prior to the date of your event.

How To Complete This Questionnaire

There are just five sections to this questionnaire and it is designed to gather information in a manner convenient to you. You don't have to complete all sections in one sitting. After you complete any of the sections you can save your work with one click and then log in at a later time to complete the rest. The total estimated time to complete all five sections is about 15 minutes.
